Heartland Community Health Center seeking community input to improve health

Heartland Community Health Center is seeking community input through an online survey to improve the health of the community through outreach and communication efforts.

HCHC is working with the University of Kansas? Center for Excellence in Health Communications to Underserved Populations (CEHCUP) for the duration of the project through a grant from the Douglas County Community Foundation and is currently doing research to measure its baseline.

HCHC?s goal for survey collection is 500, and it will use the results of the survey to design and implement an outreach and communication plan.
